After uncovering a bug Testers generate a formal defect report.

The purpose of a defect report is to state the problem as clearly as possible so that developers can replicate the defect easily and fix it.

It is highly uncommon these days that testers create a document these days.

Instead, they simply log a Defect work item in the project's ALM system.

In which case the Defect Report – if any – is created from a filtered view of logged Defects Work Items.
